Page Design of "Contact Us"

Elements that go to make up a "Contact Us" page.

I looked at 5 cities' contact us page and this what they have in common.

Central Address (on 4 site)
Central Phone (on 5 site)
Central Email (on 2 site)
Hours of Operation (on 2 site)
Links to FAQ (on 1 site)
discussion of Customer Service Center (on 1 site)
online contact form (on 1 site)
list of departmental "Contact Us" page (on 2 site) example http://www.tampagov.net/contact_us/index.asp
Emergency Numbers (on 2 site)
"Comments or concerns about the City of Chesapeake government" (on 1 site)
"Questions or comments about the City of Chesapeake website" (on 1 site)
Mayor Contact (on 1 site)
webmaster contact (on 2 site)
map of city hall (on 1 site)
Service Request System (on 1 site)
building Maps (on 1 site)

bad information on a contact us site would be an address that does not handle contact information. An example of this would be the city of Virginia Beach who puts the Web Team Address on every page. I believe that people are actually trying to get to customer service not trying to get the web team when they come to this type of page.
